Please read this Agreement carefully as it sets forth the important terms governing your use of services provided by 100% Chiropractic Central Park (“we,” “us,” or “our”).
In this Agreement, the terms “you” and “yours” refer to the individual using our services, or, in the case of use on behalf of a minor, both the parent or legal guardian providing consent and the minor receiving services.
Acceptance of Terms
Your access to and use of our services is subject to this Agreement and all applicable laws. If you do not agree to be bound by this Agreement, you may not access, register with, or use our services.
We reserve the right to revise this Agreement at any time without prior notice. Updates are effective upon posting. Continued use of our services constitutes your acceptance of any modified terms.
Your Relationship with Us
By using our services, you acknowledge that:
100% Chiropractic Central Park provides chiropractic and wellness services through licensed providers.
We may also make available non-medical products or wellness programs.
Our team of licensed professionals is solely responsible for directing the care and services they provide.
By using our services, you are establishing a direct client relationship with 100% Chiropractic Central Park, but you are not creating a provider-patient relationship with any entity beyond those directly delivering care.
Financial Responsibility
Our services are offered on a cash basis. We are not enrolled with, and do not accept, any federal or state healthcare programs (e.g., Medicare, Medicaid). You are solely responsible for payment of all services and products provided.
Subscriptions and Packages
Certain services may be sold on a recurring or membership basis. Unless canceled in accordance with our cancellation policy, subscription or membership payments will be automatically charged at the agreed interval.
You may cancel by contacting our office directly. Cancellation will be effective at the end of the current billing cycle. Refunds are not provided for partial periods unless otherwise required by law.
Use of Services
You agree to use our services only for lawful purposes. You may not:
Interfere with the operation of our services or systems.
Provide false or misleading information.
Attempt to gain unauthorized access to our systems or accounts.
Use our services for any unlawful, harmful, or fraudulent purpose.
We reserve the right to suspend or terminate access for violations of these terms.
Privacy Policy & Health Information
We respect the confidentiality of your personal and health information. Information you provide is used in accordance with our Privacy Policy and applicable laws.
Some information you provide may be considered protected health information under applicable state or federal law. Such information will be used and disclosed only as permitted by law and in connection with providing your care.
Accounts & Security

Ownership & Intellectual Property

Limitation of Liability
To the fullest extent permitted by law, 100% Chiropractic Central Park shall not be liable for any indirect, incidental, special, or consequential damages, including loss of profits or data, personal injury, or claims arising from your use of our services.
Arbitration & Governing Law
Any dispute arising out of this Agreement will be resolved through confidential binding arbitration rather than in court, except where prohibited by law. Arbitration will be conducted in Denver, Colorado, under applicable arbitration rules.
This Agreement is governed by the laws of the State of Colorado, without regard to conflict of laws principles.
Entire Agreement
This Agreement, along with our Privacy Policy and any other posted policies, constitutes the entire agreement between you and 100% Chiropractic Central Park.
